1. Presentation:

Each film-coated tablet contains:

Available as oral tablets for the treatment of various bacterial infections.


2. Description:

Azinomax 500 belongs to the macrolide subclass of antibiotics known as azalides.

It works by binding to the 50S ribosomal subunit of susceptible bacteria, inhibiting microbial protein synthesis and thereby stopping bacterial growth.

In vitro studies have shown that Azinomax 500 is effective against:

  • Gram-positive and gram-negative bacteria such as Staphylococcus aureus and Streptococcus species

  • Moraxella catarrhalis, Escherichia coli, Bordetella pertussis, Borrelia burgdorferi, Neisseria gonorrhoeae, and Haemophilus ducreyi

  • Anaerobes like Bacteroides fragilis

  • Other pathogens including Campylobacter species, Toxoplasma gondii, Treponema pallidum, Mycoplasma pneumoniae, and Legionella pneumophila


3. Indications:

Azinomax 500 is indicated for infections caused by susceptible organisms in:

  • Upper respiratory tract: sinusitis, pharyngitis/tonsillitis

  • Lower respiratory tract: bronchitis, pneumonia

  • Ear infections: otitis media

  • Skin and soft tissue infections

  • Sexually transmitted diseases: treatment of non-gonococcal urethritis and cervicitis caused by Chlamydia trachomatis in men and women


4. Dosage & Administration:

Adults:

  • 500 mg once daily orally for 3 days, or

  • 500 mg on day 1, then 250 mg once daily on days 2–5

  • For sexually transmitted infections caused by Chlamydia trachomatis:

    • 1 g as a single dose, or

    • 500 mg on day 1 followed by 250 mg daily for the next 2 days

Children:

  • 10 mg/kg body weight once daily for 3 days for children over 6 months

  • 200 mg (1 teaspoon) for 3 days if body weight is 15–25 kg

  • 300 mg (1½ teaspoons) for 3 days if body weight is 26–35 kg

  • 400 mg (2 teaspoons) for 3 days if body weight is 36–45 kg

Typhoid Fever:

  • 500 mg (2½ teaspoons) once daily for 7–10 days

Administration Instructions:

  • Take at least 1 hour before or 2 hours after meals

  • For suspension: shake bottle, add boiled and cooled water to watermark, mix thoroughly


5. Side Effects:

Azinomax 500 is generally well-tolerated. Low-frequency side effects may include:

  • Nausea, vomiting, abdominal pain or cramps

  • Diarrhea, flatulence

  • Headaches

  • Skin rashes

Most side effects are reversible upon discontinuation.


6. Precautions & Warnings:

  • Monitor for signs of superinfection with non-susceptible organisms such as fungi

  • Use with caution in patients with renal impairment (no dose adjustment generally required)

  • Avoid concomitant use with ergot derivatives due to risk of ergotism

  • May increase plasma levels of cyclosporin and digoxin; monitor co-administration


7. Use in Pregnancy & Lactation:

Pregnancy:

  • Category B

  • No evidence of fetal harm in animal studies

  • Use only if clearly needed and no safer alternatives are available

Lactation:

  • Unknown if excreted in breast milk

  • Use with caution; weigh benefits to mother against potential risk to child


8. Overdose:

Symptoms may include: hearing loss, severe nausea, vomiting, and diarrhea.

Management:

  • Gastric lavage if recently ingested

  • Supportive and symptomatic care as required

  • Keep dry and protected from heat and light

  • Store out of reach of children
