1. Indications:

Temper Extra is indicated in the following conditions —

  • Headache

  • Migraine

  • Toothache

  • Neuralgia

  • Feverishness

  • Period pain

  • Sore throat

  • Backache

  • Aches and pains associated with colds and flu

2. Pharmacology:

Temper Extra combines Paracetamol and Caffeine.

  • Paracetamol provides analgesic and antipyretic effects with mild anti-inflammatory activity.

  • Caffeine, a xanthine derivative, enhances the absorption and action of Paracetamol by increasing solubility and transmembrane permeability.

  • Caffeine also raises the pain threshold, improves alertness, and constricts dilated blood vessels in the brain—helpful in relieving headache and migraine.


3. Dosage & Administration:

  • Adults: 1–2 tablets every 4–6 hours as needed.

    • Maximum dose: 8 tablets in 24 hours.

  • Children: Not recommended for children under 12 years.

4. Drug Interactions:

  • May reduce serum levels with anticonvulsants (e.g., phenytoin, barbiturates, carbamazepine).

  • May enhance the anticoagulant effect of warfarin and other coumarins with prolonged use.

  • Absorption increased with metoclopramide and domperidone.

  • Serum levels increased with probenecid and chloramphenicol.

  • Reduced absorption with colestyramine (if taken within 1 hour).

  • May cause severe hypothermia with phenothiazines.


5. Contraindications:

  • Severe hepatic disease or viral hepatitis.

  • Severe renal impairment.

  • Known hypersensitivity to Paracetamol or Caffeine.


6. Side Effects:

Generally well tolerated.

  • Rare: skin rashes, allergic reactions.

  • Occasional: nausea, abdominal pain, dizziness.

  • Very rare: blood disorders (thrombocytopenia, leukopenia, agranulocytosis).

  • Prolonged use may cause insomnia or restlessness due to caffeine.


7. Pregnancy & Lactation:

  • Pregnancy: Use only under medical supervision.

  • Lactation: Safe to use while breastfeeding in recommended doses.


8. Precautions & Warnings:

  • Use cautiously in patients with liver or kidney impairment.

  • Avoid prolonged or excessive use without consulting a physician.

  • Do not combine with other Paracetamol-containing products.


9. Overdose Effects:

  • Early symptoms (within 24 hours): pallor, nausea, vomiting, anorexia, and abdominal pain.

  • Severe overdose may cause liver damage within 12–40 hours.

  • Medical attention and acetylcysteine therapy may be required.
