1. Presentation:

  • Oral Dosage Form: 500 mg tablets

  • Extended-release Tablets: 500 mg once or twice daily as required

  • Injectable Dosage Form: IV infusion

  • Strength: 500 mg per tablet


2. Description / Pharmacology:

Ciproking contains Ciprofloxacin, a synthetic quinolone antibacterial agent with broad-spectrum activity.

  • Active against most Gram-negative aerobic bacteria, including Enterobacteriaceae and Pseudomonas aeruginosa.

  • Effective against certain Gram-positive aerobic bacteria, including penicillinase-producing and methicillin-resistant staphylococci.

  • Bactericidal action results from interference with DNA gyrase, required for bacterial DNA synthesis.

  • Rapidly and well absorbed orally; widely distributed into body tissues and fluids.

  • Half-life: ~3.5 hours; 30–50% excreted in urine within 24 hours as unchanged drug or active metabolites.


3. Indications:

Ciproking 500 mg is indicated for:

  • Single or mixed infections caused by susceptible organisms

  • Infections resistant to other antibiotics (Aminoglycosides, Penicillins, Cephalosporins)

  • Urinary tract infections, respiratory tract infections, gonorrhea, skin and soft tissue infections, bone and joint infections, intra-abdominal infections, sinusitis, chronic bacterial prostatitis

Use only under the supervision of a registered physician.


4. Dosage & Administration:

Oral Tablets (500 mg standard):

  • Adults: 500 mg twice daily, depending on infection type and severity

  • Urinary tract infections: 500 mg twice daily

  • Respiratory infections: 500 mg twice daily; adjust as needed for Streptococcus pneumoniae

  • Gonorrhea: Single dose 500 mg

  • Other infections: 500 mg twice daily

  • Cystic fibrosis with Pseudomonas: 500 mg twice daily

Extended-release Tablets:

  • Uncomplicated urinary tract infection: 500 mg once daily for 3 days

IV Infusion:

  • Administered according to infection type, severity, and patient characteristics (follow standard 500 mg IV regimens)

  • Check IV bags for leaks, cloudiness, or precipitates; follow standard infusion protocol

Special Populations:

  • Elderly: No dosage adjustment generally needed

  • Impaired renal function: Reduce total daily dose by half if serum creatinine >265 μmol/L or creatinine clearance <20 mL/min

  • Children & Adolescents: Generally not recommended due to potential arthropathy

Continue treatment 3 days after resolution of symptoms unless otherwise directed.


5. Interaction:

  • Avoid concurrent administration with magnesium or aluminum-containing antacids, sucralfate, calcium, iron, or zinc supplements (take 2 hours after or 6 hours before Ciproking)

  • Avoid concurrent intake with milk or yogurt alone; dietary calcium as part of a meal has minimal effect


6. Contraindications:

  • Hypersensitivity to Ciprofloxacin or other quinolones


7. Side Effects:

  • Gastrointestinal: nausea, diarrhea, vomiting, dyspepsia, abdominal pain

  • CNS: headache, dizziness, fatigue, confusion, seizures, risk of retinal detachment

  • Hypersensitivity: skin rash, pruritus, rarely Stevens-Johnson syndrome or toxic epidermal necrolysis

  • Other: joint pain, mild photosensitivity, transient liver enzyme elevation, serum bilirubin, urea, creatinine changes

  • Rare: hypoglycemia, mental health effects


8. Pregnancy & Lactation:

  • Animal studies show no teratogenicity, but use in pregnancy not recommended due to risk of arthropathy

  • Excreted in milk; avoid during breastfeeding


9. Precautions & Warnings:

  • Use with caution in patients with CNS disorders, arteriosclerosis, or epilepsy

  • Encourage liberal fluid intake during therapy

  • Avoid interactions with antacids, sucralfate, and mineral supplements

  • Can be taken with or without meals (except dairy alone)


10. Overdose Effects:

  • Symptoms: seizures, hallucinations, confusion, abdominal discomfort, renal and hepatic impairment, crystalluria, hematuria, reversible renal toxicity

  • Supportive treatment recommended


11. Therapeutic Class:

  • 4-Quinolone antibacterial, Anti-diarrhoeal antimicrobial
