1. Indications:

Rupacid is indicated for the symptomatic treatment of —

  • Seasonal & Perennial Allergic Rhinitis

  • Urticaria

2. Pharmacology:

Rupatadine is a long-acting, non-sedative H1-receptor antagonist. It also antagonizes platelet activating factor (PAF).

Mechanism of action:

  • Histamine and PAF cause bronchoconstriction and increased vascular permeability.

  • Rupatadine inhibits these effects, providing superior anti-allergic action compared to isolated antihistamines.

  • Inhibits mast cell degranulation and cytokine release (e.g., TNF-Îą), reducing inflammation.


3. Dosage & Administration:

Adults and adolescents (>12 years):

  • 10 mg once daily, with or without food.

Children 2–11 years:

  • â‰Ĩ25 kg: 5 ml (1 teaspoon) oral solution once daily

  • 10–<25 kg: 2.5 ml (ÂŊ teaspoon) oral solution once daily

4. Drug Interactions:

  • With medications: Concomitant use with ketoconazole or erythromycin increases systemic exposure. Use caution with CYP3A4 inhibitors, statins, CNS depressants, or alcohol.

  • With food: Avoid grapefruit or grapefruit juice while taking Rupatadine.


5. Contraindications:

  • Hypersensitivity to Rupatadine or any excipients.


6. Side Effects:

Common:

  • Asthenia, dizziness, drowsiness

Uncommon:

  • Appetite changes, arthralgia, back pain, concentration issues

  • Constipation, cough, diarrhea, dry throat, epistaxis, fever

  • Gastrointestinal discomfort, infection risk, irritability, malaise, myalgia

  • Nasal dryness, nausea, oropharyngeal pain, rash, thirst, vomiting, weight gain

Rare:

  • Palpitations, tachycardia


7. Pregnancy & Lactation:

  • Pregnancy: Avoid unless benefits outweigh risks; no clinical data available.

  • Lactation: Unknown if excreted in breast milk; do not use unless necessary.


8. Precautions & Warnings:

  • Use caution in patients with prolonged QT interval, uncorrected hypokalemia, or proarrhythmic conditions.

  • Elderly patients (â‰Ĩ65 years) should use with caution.

  • Not recommended for patients with renal or hepatic impairment due to lack of data.


9. Use in Special Populations:

  • Elderly: Use with caution; no dose adjustment indicated.
